The International Foundation of Integrated Care (IFIC) in partnership with the Health Service Executive (HSE) presents the 17th International conference on Integrated Care “Building a platform for integrated care: delivering change that matters to people” which will take place in Dublin, Ireland 08 to 10 May 2017. The meeting is in association with 5th World Congress on Integrated Care (WCIC5).
The conference will bring together researchers, clinicians and managers from around the world who are engaged in the design and delivery of integrated health and social care. They will share experience and the latest evidence about integrating Public Health, Health and Social Care and the New roles and Possibilities for Hospitals, producing Positive and Curative Integrated Mental and Physical Care, mobilising key enablers like policy making and Mobile and Digital Health Solutions, and investment in an Integrated Care Workforce, clinical leadership and coproduction with individuals, careers, communities and populations.

Submit an Abstract
The scientific committee has been established from international experts who are leading in the field of integrated care policy and practice, and they are now accepting paper submissions relating to the conference themes until Friday, 16 December. The paper should take the form of an abstract of not more than 500 words. Abstract Submission Guidelines are available to download from the conference website.
All accepted abstracts will be published in the International Journal of Integrated Care (Impact Factor 1.557) Accepted abstracts will appear in the programme as a formal oral presentation, workshop or poster, as deemed appropriate by the scientific committee. All accepted abstracts will compete for the Annual IFIC Integrated Care Awards. ICIC16 in Barcelona received over 500 submissions so we advise that you take due consideration when submitting your paper to ensure you have the very best chance of success.
